What Vital Records can be found in Madison?

Vital Records in the Town of Madison include births, deaths & marriages that date back to May 5, 1826.

Prior Certificates would be found in Guilford CT, as we were East Guilford at that time.

What is the fee for obtaining a Vital Record?

Certified copies of Vital Records (Births, Deaths, Marriage, and Civil Unions) registered in the Town of Madison may be purchased for a fee of $10 per copy to any eligible party.

How do I request a copy of a Vital Record?

Vitals Records may be requested in person or by mail.

For copies by mail, send a copy of a photo ID, along with the fee of $10 per certificate, checks ONLY, and the completed Request Form.

You must include a stamped self-addressed envelope for return of the certificate(s).
