21-28.  21 Scenic Drive.  Map 115, Lot 68.  Owner:  Harold & Mary Annicelli; Applicant:  Michael Camarota, Camarota Sanitation & Excavation. Regulated Activity Permit for a septic system failure/repair in the same location in the review area.
