Good evening. The is First Selectwoman Peggy Lyons with important information for Madison Seniors. The Madison Senior Center is launching a survey of residents 65 years and older who are interested in receiving the COVID-19 vaccination. Starting tomorrow, January 14th, we are asking any Madison resident in this age category who wants to receive the vaccine to please complete the survey located on the Town website. If you do not have access to the internet or encounter issues when completing the survey, please contact the Senior Center at (203) 245–5627. Information collected from the survey will be used to provide a contact list for future vaccine administration plans.

Please note you will NOT be registered for the vaccine by completing the survey. However, the Town will be using this list to ensure that those who qualify for Phase 1b get the most timely, up-to-date information and instructions. Currently, residents 75 years of age and older are included in Phase 1b, but this is subject to change and likely to include additional age groups. Once more specific details for Phase 1b eligibility and roll-out are finalized, the Town of Madison will reach out to you with an update. This will be separate from any communication or registration administered by your healthcare provider or the State of Connecticut.
We also encourage all Madison residents to sign up for the Town’s new communication system, Madison Alerts, at www.madisonct.org/madisonalerts.
